Restorative Measures application and registration of interest - for persons of sufficient interest

This form enables you to apply or register your interest for restorative measures on behalf of a deceased veteran who was impacted by the policy prohibiting LGBT Personnel serving in the Armed Forces between 1967 and 2000.

Please note, you will need to provide some identifying information for the veteran to enable Defence to locate their Service records. This includes the veteran's Service number.

Some restorative measures are ready to be awarded immediately while others will take longer. This form includes the restorative measures that will be ready soon. The Defence Restorative Action team will deliver these as soon as they're available. You do not need to apply again.

The dedicated LGBT Veterans: Support and next steps webpage will provide the latest information on when restorative measures become available.

Please indicate the measures that you believe the person you are applying on behalf of are eligible for.

Please ensure your information is accurate as errors will cause a delay in processing. Please email your evidence upon submitting your application. This may include; proof of name change, proof of connection to a deceased veteran or a testimony to be added to those given in the Review and submitted to the National Archives.

Please only complete the form once.
